	The personal data provided in this application will be used for applying for a subsidy under the Greening Hong Kong Activities Subsidy Scheme and other matters in relation to the application. The provision of personal data in this form is voluntary. If you do not provide sufficient information, the Government may not be able to process your application or correspond with you.

	The personal data you provided in this form may be disclosed to other government bureaux or departments for cross reference should the need arise.
3. Access to and Change of Personal Data
	You have a right of access and correction with respect to personal data as provided for by Personal Data (Privacy) Ordinance (Cap.486). Your right of access includes the right to obtain a copy of your personal data given in this form. In case of need, please write to the Green Campaign Section at the address as below-mentioned. Upon verification of signature and official chop, the request will be followed up.
