Prospectuses and Enrolment Forms
   
(1) Prospectuses and enrolment forms will be available on the first day of enrolment at the following locations:
 
a) District Leisure Services Offices (DLSOs) of the Leisure and Cultural Services Department (LCSD)
b) Any recreational and sports venues with Leisure Link Services

(3) The completed enrolment form (attachments or presentation of identity documents is not required when submitting application) should be submitted within the above-mentioned enrolment period:
 
a) In person to : DLSOs of the LCSD (within office hours) or any recreational and sports venues with Leisure Link services ; or
b) By post to :
Major Events Section, LCSD,
2/F, Leisure and Cultural Services Headquarters, 1-3 Pai Tau Street, Shatin; or 2/F, Leisure and Cultural Services Headquarters, 1-3 Pai Tau Street, Shatin; or
c) click here for e-submission of enrolment form
 
(4) Participants / teams will be selected by balloting if the application exceed the quotas.
 
(5) Points to Note
 
a) Information provided by participants will only be used for the purposes of enrolment, compilation of statistics, future liaison works and promotion of activities organised by LCSD. Apart from staff duly authorized by LCSD, no one will be given access to the personal information.
b) If participants want to revise or retrieve the personal information reported, participants may contact the staff of Major Events Section, LCSD.
c) The provision of participants�� personal information is obligatory. If the requisite personal information is not provided, LCSD may not be able to process the application.
d) The personal information of participants will be destroyed within 3 months after the completion of the Masters Games 2012/13.
e) The balloting results of successful applicants and applicants of the waiting list will be displayed at the DLSOs of the LCSD any recreational and sports venues with Leisure Link services and the LCSD Webpage.
f) The competition schedules, fixtures and competition results will also be displayed at the LCSD webpage. Only the Chinese and English names of the participants will be disclosed on the information mentioned above.
