Period for submission of Enrolment Form

: 27 May – 7 June 2010
 
Enrolment :
1.

Completed enrolment forms should be submitted during the submission period. (originals/copies of identification documents need not be attached or produced when the forms are submitted)

2.

Each participant may participate in a maximum of two events, i.e. one singles event and one doubles event. He/She shall submit one enrolment form for each event only. The LCSD reserves the right to disqualify any participant who has made repeated enrolments or has submitted an incomplete enrolment form.

3.

Participants will be selected by ballot.

4.

The ballot will be held on 22 June 2010 at 4:30pm at Audio Visual Room, 1/F., Leisure and Cultural Services Headquarters, 1-3 Pai Tau Street, Shatin. Members of the Public are welcome to attend.

The balloting results (only the Chinese and English names of participants will be disclosed) will be announced at the DLSOs of the LCSD, any recreational and sports venues with Leisure Link services and on the LCSD Masters Games webpage on 25 June 2010. Notification letters will be sent to successful applicants and applicants on the waiting list (no priority number will be assigned to applicants on the waiting list) while unsuccessful applicants will not be notified.

Successful applicants shall bring along their notification letters/identity documents (originals or copies) and entry fees to the DLSOs of the LCSD (during office hours) or any recreational and sports venues with Leisure Link services for verification of their personal particulars and payment of entry fees during the specified payment period. Successful applicants who fail to pay the required fees within the specified payment period will be regarded as waiving their rights. The unfilled places will be allocated to the applicants on the waiting list. Enrolment on a first-come-first-served basis will be accepted during the specified payment period for the applicants on the waiting list.

7.

If there are still unfilled places in individual divisions after the payment period for the applicants on the waiting list, or the number of submitted applications is less than the regular quota, the remaining quota will be open to the public for enrolment during the following period at DLSOs on a first-come-first-served basis.

8.
For participants in doubles events, both players in a team shall pay the entry fees, or else the team will be regarded as waiving their rights of participation.
