(2)	The completed enrolment form (attachments or presentation of identity documents is not required when submitting application) should be submitted within the above-mentioned submission date :
	a) In person to : District Leisure Services Offices of Leisure and Cultural Services Department (LCSD) (within office hours) or any recreational and sports venues with Leisure Link services; or b) By post to : Major Events Section, Leisure and Cultural Services Department, 2/F, Leisure and Cultural Services Headquarters, 1-3 Pai Tau Street, Shatin; or c) click here for e-submission of enrolment form
(3)	Participants / teams will be selected by balloting.
Points to Note 1. Information provided by participants / teams will only be used for the purposes of enrolment, compilation of statistics, future liaison works and promotion of activities organised by LCSD. Apart from staff duly authorized by LCSD, no one will be given access to the personal information. 2. If participants / teams want to revise or retrieve the personal information reported, participants / teams may contact the staff of Major Events Section, LCSD. 3. The provision of participants’ / teams’ personal information is obligatory. If the requisite personal information is not provided, LCSD may not be able to process the application. 4. The balloting results will be displayed at the District Leisure Services Offices of LCSD, any recreational and sports venues with Leisure Link services and the LCSD Webpage. The competition schedules, competition fixtures and results will also be displayed at the LCSD webpage. The Chinese and English names of the participants / teams will be disclosed on the above-mentioned balloting results, competition schedules, competition fixtures and results.
