Please fax, send or bring in person the following materials to the office during office hours*on or before the deadline for application:

1. Application

• A completed application form. (Only one application form is accepted for each person and each course)
• Incomplete application form will not be considered. There is no need to submit
  cheque during the application period.

2. Fee

• Applicants will be notified by post of the result and method of payment.
• Successful applicants should submit a separate crossed cheque for each course,
  payable to "The Government of HKSAR", before the date stated in the notification
  letter.
• Fees are not refundable or transferable.

3. Quota

• In case of over application, participants will be decided by lot drawing.
• If there are vacancies after the deadline, we will accept applications on a first come, first served basis until the quota is full.
• The Museum reserves the right to cancel the programmes if the number of applications is below the minimum requirement.
• Due to course arrangements and the safety use of art studios, applicants with disabilities are welcome to call us beforehand for course details / inform us of any special needs.

The personal data provided serves only for application process. The applicant has the right to request access to and to request the correction of personal data as stated in the form in accordance with Section 18, 22 and Principle 6 of Schedule 1 to the Personal Data (Privacy) Ordinance.
