2013 Hong Kong Youth Music Camp - Day Camp

	Day Camp Fees

	$59 per person (including lunch) Concessionary rate at $43 per person (including lunch) for children, local school/ full-time students, senior citizens, people with disabilities and participants from subvented organizations registered with Social Welfare Department. (Please refer to the application form for details)

	Application

	Application deadline: 20 May The completed application form with 2 stamped ($1.40) and self-addressed envelopes should be sent to: Kwun Tong Music Centre 4/F Lam Tin Complex, 1 Hing Tin Street, Lam Tin, Kowloon (Please state “Day Camp Application” on the envelop) An acknowledgement slip will be issued upon receipt of application, please call 27961003 if it is not received by 23 May. In case applications exceed the quota, allocation of places will be determined by ballot. Balloting will be held in Music Office Kwun Tong Music Centre on 24 May at 11am. List of successful applicants will be available at the official website: www.lcsd.gov.hk/musicoffice from 30 May onwards. Successful applicants will also be notified by mail by 31 May. (Families of the residential campers can choose to join the Day Camp after announcement of the recruitment results in late May) Click HERE to download the application form

Payment

Successful applicants should submit the following item(s) to the Kwun Tong Music Centre (at the address above) by 3 June, otherwise, they will be deemed to have given up their applications.

A crossed cheque for payment of camp fee should be made payable to the "Government of the Hong Kong Special Administrative Region". Please also state the name of applicant, the registration no. and the no. of campers of full rate & concessionary rate at the back of the cheque. Postdated cheques will not be accepted. Except under the condition as set out in Remark 8, camp fees paid are non-refundable and non-transferable.
A stamped ($1.40) and self-addressed envelop (for applicants who send the cheques by mail only). Receipts will be issued to successful applicants upon payment of camp fees, please check with the Music Office immediately if a receipt is not received by 10 June. All day campers should show their receipts on entering the campsite.
